Stimulants methamphetamine and amphetemines ( adderal and ritalin), cocaine, marijuana, PCP and the designer drugs, ( ecstasy, GBH, ketamine).

Detoxification of people having mild withdrawal symptoms from alcohol or drugs and people using cocaine, marijuana, opioids, or methamphetamine, do not generally need to be hospitalized for detoxification. However they may need a referral for medical assessment, evaluation and medications to assist with the discomfort of withdrawal and associated symptoms. It is essential that a person seeking to ‘detox’ from alcohol and / or drugs be evaluated by a physician trained in Addiction Medicine.

“The detox off opiates was less drastic with Suboxone, compared to other drugs that I’ve tried. The physical symptoms are minimal.” R.J. H.

After the detoxification process is complete, you will discuss your options with the Clinical Team.
This would include recommendations for continued rehabilitation in residential or outpatient or extended care.

Anti- Craving Medications

The Clinical Team can explain any recommendations for some of the newer Anti- Craving Medications, if you have had trouble staying off drugs after detox in the past. Some of these are:

Campral is a delayed release tablet used for the maintenance of abstinence from alcohol in individuals, with alcohol dependence. Campral helps reduce the physical and emotional discomfort many people feel in the weeks and months after they have stopped drinking and participating in treatment. Campral has demonstrated success,
In helping clients maintain long-term sobriety.

Naltrexone (Revia) is a safe, effective oral medication used as part of a comprehensive treatment program as an aid to treat alcohol dependence and opioid addiction. While effective, it must always be remembered that naltrexone is a tool to assist an individual in the process of recovery, and is not a cure in and of itself. Naltrexone is an opiate blocker and prevents all opiate addicts from getting high and reduces craving in the alcoholic.

Vivitrol is Naltrexone in injection form. It lasts approximately a month and helps people with compliance.

Antabuse is an antagonist medication, usually used to assist a chronic alcoholic; it acts as a deterrent, creating an unpleasant physical response to drinking.

Since relapse prevention is the focus of treatment, Cornerstone recommends that our affiliated physicians consider these anti-craving medications for our residents, as part of building a solid foundation for recovery.

You may be given a recommendation for an evaluation by an independent psychiatrist to assess
co-occurring, psychiatric/psychological problems and the possible need for medications. If the psychiatrist is not covered by your insurance, then you will be expected to pay for the consult.

Additional Information about Detoxification

Detoxification is not substance abuse treatment and rehabilitation. David Smith, M.D., recent Medical Director of the Haight Ashbury Clinic in San Francisco said “When there is physical dependence, medical interventions may be needed to counter withdrawal symptoms and make full detoxification possible. The tools of detoxification include a pharmacopeia of medications that work to ease withdrawal symptoms and help the individual’s system regain a healthy balance.”

The definition of detoxification is: a set of interventions aimed at managing acute intoxication and withdrawal. It is a clearing of toxins from the body of someone who is acutely intoxicated and / or dependent on substances of abuse.

Detoxification seeks to minimize the physical harm caused by the abuse of substances. Supervised detoxification may prevent potentially life-threatening complications that might appear if the person was left untreated.

Each potential client will need:

Evaluation / Assessment entails testing for the presence and concentration of substances of abuse in the urine, blood alcohol level done by breathalyzer and screening for co-occurring mental and physical conditions. Evaluation also includes a comprehensive assessment of the individual’s medical and psychological history and social situation to help determine the appropriate level of treatment following detoxification.
Essentially, the evaluation serves as the basis for the initial substance abuse treatment plan once the individual has been withdrawn successfully.

Stabilization includes the psycho-social and medical processes of assisting the individual through their symptoms from their alcohol or drug withdrawal, to a medically stable, fully supported substance-free state. This often is done with the assistance of medications, though not all individuals require medication for their detoxification process. Each person is evaluated, and given a personalized regime which depends on what substances were used, how long they were used, how they responded in past withdrawal situations and what other medical complications might be expected. Every person’s detox at Cornerstone of Southern California is very individualized and unique. Stabilization includes familiarizing a person with what to expect in treatment and recovery. During this time the clinical staff will ask for involvement from the family and others significant to the resident.

Educating the client involves preparing the individual for the next stage in their substance abuse treatment, stressing the importance of following through with the complete continuum of care.

All three components (evaluation, stabilization, and education) involve treating the individual with compassion and understanding. Individuals undergoing detoxification need to know that someone cares about them, respects them as individuals, and has hope for their future. The kind of care taken during the detoxification process will show the person that the recommendations being made can be trusted and followed.
